Developing a holistic, patient-centred intervention

Some elderly patients have complex healthcare needs that are difficult to treat and manage. The ESCAPE project aims to improve physical and mental health outcomes for multimorbid elderly patients through integrated and personalised treatment plans. Using digital health technologies, ESCAPE will create integrated and personalised treatment plans for elderly patients with multiple health conditions with the aim of ultimately improving their physical and mental health.

Multimorbid patients have complex healthcare needs. Multimorbidity is common in seniors with three out of four suffering, making it a key global health challenge.

Bringing together partners from eight countries across Europe

The interdisciplinary team includes experts from general and hospital medical practices, psychology, health economics, e-Health systems, and patient and informal carer representatives.
